The first and second stages fall behind because: (1) the next stage ignites and accelerates away (2) the next stage has solid rocket motors that fire during stage separation to allow more room faster before ignition of its engine. The LM bay panels are released with explosive bolts and spring mechanisms. Their trajectory diverges from the CM/LM less severely than staging. You can see these things from a great distance against the backness of space.

Search YouTube for Shuttle External Tank and you will see some very nice footage from STS 121 of the fuel tank tagging along after they reached orbit. As you can see, they had a good opportunity to observe it for a long time, which has been very useful with the insulation problems they've been having.

You make a very good point indeed and are very well versed in apollo stage seperation procedure but the question remains ,how could conrad make such a glaring misidentification of a peice of his own craft ..it just doesnt make sense.

So, who says it was a misidentification? He called it a "Bogey", right? He's a fighter pilot. He's just using off the cuff jargin that he and the other fighter jocks at Houston Control understand. Probably a bit of irony in the statement, because... you know... Who else is out there? He didn't say, "there's an extraterrestrial spacecraft on an intercepting with us." He saw something and reported it. That's his job.

Three more reasons that the first two stages fall behind after stage separation... (3) They have not reached orbital velocity. (4) Their engines have shut down. (5) They are not yet entirely free of the atmosphere.

As for positively identifying what it actually was, it must have been far enough away that he couldn't tell what it was, only that something was out there that was behaving differently than their celestial companions and the usual debris. It might have been one of the interstage pieces or something.

All he say's is, We have a bogey. Now, I'm no astronaut, but I can imagine being stuck inside a spacesuit with no tissues to wipe off the offending bogey, clearly a very annoying experience. Only thing you can do is tell Huston.
